For new OCI application and renunciation of Indian passport,follow the steps below.Send 2 separate package of documents-One is for OCI application and one is for renunciation of Indian passport. We have to mail original US passport and it will be returned in 2 weeks. OCI card processing is roughly 4 months, so if we have to travel to India prior to that we can get Indian Visa online and travel. If you get stuck anytime in the process call CKGS customer service number and they are very helpful.
STEP1:
Go to Fedex/CVS/Walgreens and get 4 passport size photos. We need only 2 copies but keep 2 as backup. Also take a picture of the passport size picture in your cellphone and save a digital copy.
STEP2:

# Spouse's Indian and US Passport first 3 pages and last 2 pages copy
# Scan and keep copy of your family members existing OCI cards if any

STEP3:
Go to https://www.in.ckgs.us/oci/ website. Apply for new OCI card. For Payment mode we can only choose Bankers check or money order option. Credit card payment option has been removed recently for some reason.
STEP4:
Go to https://ociservices.gov.in/
Upload scanned copy of US and Indian passport information page only and make sure it is less than 1000KB size. Also upload DL, Marriage certificate copy.

This is important step:-Link Government web reference number to CKGS web reference number
STEP 6:
# Go to Bank and get a cashier's check or money order for the final amount in CKGS website. Address the check or money order to "COX AND KINGS GLOBAL SERVICES USA LLC".
# Go to Notary (UPS store or a bank) and get the Affidavit form notarized

Login to CKGS website and print all documents listed in the website.

# Print all other supporting documents
# 2 different Fed-Ex labels and print the bar code twice and attach it at the bottom of both envelopes.
# Keep the original money order in OCI envelope and photocopy of money order in renunciation envelope.
STEP7:
Ship 2 separate envelopes. One for OCI and one for Indian Citizenship renunciation and check for updates in your email or mobile app. Print the bar code from CKGS page twice and stick it at the bottom of the label for both envelopes.
