PIO Card for Minors (Info below assumes parents are still Indian citizen and have GC or H1-B in US). PLEASE REFER to http://www.in.ckgs.us/pio/pio-categories/new-pio-minor.shtml
for detailed instruction.

I would recommend you follow the steps in the same order as below, so that you have all the required information when you are filling out the forms. I wish they had these steps for me when I was doing it, I had to go back and forth couple of times and it  took a good amount of my time to get it done.

Step 1:- Get the Money order/DD/Cashier's check ready. We need this number before printing the PIO application form. I got a Money order as we can get it in any grocery store. DD and Cashier's check are available only at the bank. They have online payment options with credit card or you can do bank transfer as well, however both options didn't work for me as they had vague information on the website at the time I filed it and online payment wasn't working either, so I opted to pay with money order instead.

Step2:- Register online with Fedex and get User ID and Password, then associate one of your credit card to the account and print a return label with your home address as destination and sender will be PIO application process center address. We need this tracking number (AWB #) before printing the CKGS form. However they have an option to tick return mail in the CKGS form, but the payment method for that option wasn't clear. So to be safe I just opted to send the return Fed-ex package along with other documents.

Step3:- Go to Fedex/CVS/Walgreens and get 4 passport size photo of the infant and keep it ready.

Step4:- Go to Staples/Office depot and buy black or blue stamp pad/fingerprint ink pad. So we can get the fingerprint of the infant in the PIO application form

Step5:- Fill out the following forms and then take color printouts
PLEASE REFER http://www.in.ckgs.us/pio/pio-categories/new-pio-minor.shtml

1.PIO Application form (Need 2 sets)- Fill it online and then print. After getting thumb impression of infant (Take multiple print outs of those pages that need infant finger prints as it is not easy to get proper finger print of the infant in one copy)  then go to notary and get it notarized and both parents should sign in front of the notary. Notary is available in any bank, UPS/Fedex store or sometimes your colleagues at your office will be a notary as well.

2.CKGS order form- Fill it online and then print. Both Parents sign the form
LINK TO CKGS FORM http://www.in.ckgs.us/resources/pdf/order-form-pio.pdf

3.Additional particulars form. For travel date simply enter 6 months timeframe from the date you are filing- Fill it online and then print.
LINK http://www.in.ckgs.us/resources/pdf/additional-particular-form.pdf

4.Parental authorization form--Print it and then hand fill it
LINK http://www.in.ckgs.us/download-forms.shtml

Step6:- Take photocopy of the below documents
Photocopy of infant USA passport pages (Refer PIO instructions page below for page numbers)
Photocopy of infant birth certificate
Photocopy of both parents - DL, Greencard/ H1-B Visa documents
Photocopy of parents Indian passport pages (Refer PIO instructions page belowfor page numbers)
LINK Go To http://www.in.ckgs.us/pio/pio-categories/new-pio-minor.shtml and click on Documents Required TAB

Once you have all the documents ready ship it with the return fed-ex label and package enclosed. Also make sure you send the original US passport of the infant along with it. In case you are traveling local within US and you have the the infant passport sent out for PIO processing, you can travel with the infant's birth certificate.

Baby Home remedies/treatments

TREATMENTS:-
This is the summary of all the information I got from friends and doctor visits.
What to do if baby gets cold or cough?
* Apply baby rub on the chest, foot and little behind the ears
* Spray nasal saline drop and use nasal bulb and keep removing the mucus 3 or 4 times a day, so that it doesn't get to the lungs
* Use cool mist humidifier in the room so that it will keep the moisture intact in the room and releive some congestion
* Riase the baby's crib bed with a cloth or something and let is sleep slightly elevated.
* As long as the baby is eating, pooping and sleeping well, then doing the above will take care of cold and cough. If for some reason baby is not eating or sleeping well then check with the doctor.
* While breastfeeding mother can eat Vitamin C rich food and can add turmeric in all her food, so that baby will get that milk.
* For older infants/Toddler-Kids above 1 1/2 year old. Add turmeric to Milk (Boil for a minute), bring it to room temperature and then add honey and mix it and filter it. Give it to them  before bed and it will reduce cough in the night.
* For older infants/Toddler-Kids above 1 1/2 year old you can make  mix ginger extract, lemon juice and honey in a small cup and give them 3 or 4 spoons of it and it will reduce cough and cold

What to do if the baby has temperature?
* Use thermometer and keep monitoring temperature frequently
* Call your doctor office and check with them
* Use wet cloth and keep it on forehead, underarms, leg folds and hand folds.
* If dehydrated give them Pedialyte,Orange juice and other fruits
* Safe food will be Idly and rasam
* Rule is less food more liquids during fever.

What to do if kids keep vomiting?
* Stay away from dairy product
* Give pedialyte, apple sauce, water or chicken broth soup
* BRAT food
* No fatty foods or oil

What to do if kids have Diarrhea?
* Carrot and Banana will constipate them and reduce diarrhea. So for infants give carrot or banana puree. For older toddlers you can make them eat it as is.
* Give Yogurt
* Floraster supplement
* Bread, Idly
* Pedialyte and keep them hydrated

IMMUNITY BOOSTER DRINKS
Got this recipe from my friend, as kids get frequently sick with daycare infection, we can try the below options to boost their immunity. FYI

Recipe 1
Javvarisi - 1 cup
Barley - 1 cup
Jeera - 1/4 teaspoon

Dry roast javvarisi nicely. Dry roast barley and cumin together. Mix these in the mixie and make a really really fine powder. Store this mixture in an air tight container.
Whenever ready, boil 1 tumbler milk (8 oz) nicely and add about 3/4 teaspoon of this mixture to the hot boiled milk. Mix it in two tumblers nicely like you mix horlicks so that there are no lumps. Add sugar or honey as required and drink it.

Recipe 2
-------------
Turmeric powder
Ginger powder
Cloves
Cardamom
Cinnamon
Whole pepper
Mix 1 teaspoon of all the above in a microwave safe bowl and microwave them for 2 minutes. Set it aside to cool for about 10 minutes. Grind these into a very fine powder (using mixie or coffee grinder). Store it in an air tight container in the freezer.
Every night, take 1/4 to 1/2 teaspoon of this powder and boil it along with 1/2 cup of water. Add milk and filter it with a tea filter. Add honey for sweetness and drink it. This will surely boost immunity.

Baby Products that I use

Here is the list of Baby products that I use:-
DISCLAIMER:- These are the products that worked for me, it may or may not work for you.

CHECK OUT THE AMAZON LIST BELOW FOR OTHER ITEMS NOT LISTED HERE

BODY WASH Cetaphil Restoderma body wash for bathing and hair wash. I tried Aveeno and Johnson and Johnson but skin was getting dry, so doctor recommended to use Cetaphil body wash.
BODY LOTION Cetaphil creme 3 to 4 times a day. After giving a shower to the baby just dab the towel on the baby very lightly and spread the moisturizing creme while the body is slightly wet from the shower.
3.  If for some reason if your baby has ECZEMA-extreme dry skin then do the following
a. During the day use Cetaphil creme 3 to 4 times. Each time slightly rinse the face, hand and legs and apply creme while it is wet.
b. In the night after shower or towel bath use Eucerin Aquaphor ointment before putting them to bed.
c. Occasionally skin will get very dry and you will see red/pink rashes, for that apply CORTIZONE-10 creme only on the spots where you see rashes and it will go off in 1 or 2 minutes.

DIAPERS
During the day time just get store brand diapers from Wal-mart/Sams etc. In the night use branded diapers like Pampers or Huggies, so that they absorb really well and baby won't wake up in the night due to wetness. Also for the first 8 months buy diapers in small quantity as you will use one size diaper for just 1 or 2 months and then they will outgrow it and you will have to move to next size.
WIPES:- Get scent free wipes if you are using wipes to clean. You can also clean the baby with water in washbasin, like we do in India instead of using wipes all the time, that way you can avoid diaper rash as cleaning with water is better than wipes. Keep wipes in diaper bag when traveling outside.
DIAPER RASH CREME DESITIN can be applied if you see diaper rash or if you are traveling apply diaper rash creme even if there is no rash coz the baby will be in the car seat for longer duration.

FEEDING
Feeding baby directly is best. Once you start working you can start using breast pump and MEDELA Pump in style pumps are really good. If you are working , your company might provide free breast pump, check with your company HR and also call your insurance company as well.
Feeding bottle I highly recommend Dr.Brown bottles as they reduce colic and gas in babies. Only issue is cleaning is a bit tiring with Dr.Brown. I usually get extra bottles and put it in the dishwasher with Sanitizing mode turned on. So if you use dishwasher it will sanitize as well, no need to buy separate electric steam sanitizer. If not you can use a big stainless steel bowl and heat water in it and put the bottles in and sanitize.
FORMULA I used ENFAMIL brand formula. Other good product is SIMILAC, you can try and see which one your baby likes.

LAUNDRY Use DREFT baby liquid for laundry

CRIB
We bought Delta 4 in 1 convertible crib VENETIAN or CANTON (Espresso or Cherry color)
MATTRESS
Colgate VISCO Foam Mattress---I recommend you to get Foam mattress instead of COIL mattress as coil mattress is really heavy and it will be difficult to change the crib sheet which you have to do frequently.

TRAVEL SYSTEM
Car seat and stroller together is called travel system. I have the Chicco model but I recommend Graco as it is much lighter. I highly recommend you take a tour both in "Babies r us" and "Buybuybaby", talk to the sales rep and ask them to explain the difference between each model and then decide.
I recommend you go for the lighter weight model so it is easy for travel.

THINGS WE DON'T HAVE TO BUY IMO
CHANGING TABLE/PAD:- I would say simply put the waterproof sheet on the floor or on our bed and change the diaper.
BATH TUB:- I prefer bathing baby putting them on our legs and using buckets of water to bath them like we do in India. I am not a big fan of American style tiny bath tubs.
NURSING BRA:- It was a hassle, I would say simply wear 2 piece pajamas and nurse. Indian feeding nighty didn't work well, so wouldn't recommend it.
PACIFIERS:- I did think it was bliss to use pacifiers but then it didn't work for the baby as the baby got  used to it and wanted it when she was falling asleep and the moment it fell out of her mouth she used to cry. So we had to frequently go and keep it back in her month until she was fully asleep. So I would say use it only if you traveling long distance and if baby is getting cranky sitting in the car for longer duration.
WIPES WARMER:- It is a great product and may be a necessity only if you are in colder places where there is severe winter with heavy snow.
SLEEPSACK:- I tried it thinking it would be useful but didn't work as the baby would wake up each time we try to put them in. So for the first few months the hospital towels worked great and after 6 months I started using the baby blanket by tucking it in on both sides of crib so that it does't come out.
